- Beef and beef products: All eligible products must be certified by an FSIS Form 9060-5: Meat and Poultry Export Certificate of Wholesomeness issued through PHIS.*
- In addition to the FSIS Form 9060-5: Meat and Poultry Export Certificate of Wholesomeness, the following statements must be provided on FSIS Form 9060-5B: Meat, Poultry And Egg Products Export Certificate of Wholesomeness Continuation Sheet:.*

The United States complies with the conditions in the Terrestrial Animal Health Code of the OIE as a country that presents a Negligible Risk for bovine spongiform encephalopathy.
The bovine meat and meat products were subject to testing in accordance with the U.S. National Residue Program.
NOTE: The export applicant will select the statements above via the PHIS statements module when completing the export application in PHIS. The selected statements will be added by PHIS to FSIS Form 9060-5B. .*
