Alabama: Poultry and poultry products derived from birds originating from the State of Alabama slaughtered on or after October 6, 2023 are ineligible.*

Alaska: Poultry and poultry products derived from birds originating from the State of Alaska slaughtered on or after August 1, 2022 and before April 10, 2023 are ineligible.*

Arkansas: Poultry and poultry products derived from birds originating from the State of Arkansas slaughtered on or after August 1, 2022 and before December 31, 2022; and on or after October 10, 2023 are ineligible.*
- California: Poultry and poultry products derived from birds originating from the State of California slaughtered on or after August 1, 2022 and before June 1, 2023; and on or after October 5, 2023 are ineligible.*
- Colorado: Poultry and poultry products derived from birds originating from the State of Colorado slaughtered on or after March 18, 2022 and before August 23, 2023 are ineligible.

- Georgia: Poultry and poultry products derived from birds originating from the State of Georgia slaughtered on or after October 31, 2023 are ineligible.*
- Idaho: Poultry and poultry products derived from birds originating from the State of Idaho slaughtered on or after March 24, 2022 and before March 23, 2023 are ineligible.
- Illinois: Poultry and poultry products derived from birds originating from the State of Illinois slaughtered on or after November 9, 2022 and before May 4, 2023 are ineligible.*
- Indiana: Poultry and poultry products derived from birds originating from the State of Indiana slaughtered between January 18, 2022 and July 20, 2022 and between August 11, 2022 and April 18,2023 are ineligible.
- Iowa: Poultry and poultry products derived from birds originating from the State of Iowa slaughtered between February 13, 2022 and August 3, 2022 and between October 10, 2022 and May 22, 2023; and on or after October 2, 2023 are ineligible.*
- Kansas: Poultry and poultry products derived from birds originating from the State of Kansas slaughtered on or after February 18, 2022 and before March 29, 2023 are ineligible.
- Kentucky: Poultry and poultry products derived from birds originating from the State of Kentucky slaughtered between January 22, 2022 and May 14, 2022 and between September 15, 2022 and November 11, 2022 are ineligible.
- Maine: Poultry and poultry products derived from birds originating from the State of Maine slaughtered on or after March 1, 2022 and before September 18, 2022 are ineligible.
- Maryland: Poultry and poultry products derived from birds originating from the State of Maryland slaughtered between February 11, 2022 and August 16, 2022 and between November 8, 2022 and January 17, 2023; and on or after October 31, 2023 are ineligible.*
- Michigan: Poultry and poultry products derived from birds originating from the State of Michigan slaughtered on or after March 23, 2022 and before July 6, 2023 are ineligible.
- Mississippi: Poultry and poultry products derived from birds originating from the State of Mississippi slaughtered between October 14, 2022 and January 6 2023, and between January 17, 2023 and May 16, 2023 are ineligible.
- Missouri: Poultry and poultry products derived from birds originating from the State of Missouri slaughtered between February 11, 2022 and July 21, 2022 and between November 1, 2022 and April 22, 2023; and on or after October 16, 2023 are ineligible.*

- North Dakota: Poultry and poultry products derived from birds originating from the State of North Dakota slaughtered on or after March 8, 2022 and before June 16, 2023; and on or after October 25, 2023 are ineligible.*
- Ohio: Poultry and poultry products derived from birds originating from the State of Ohio slaughtered on or after August 12, 2022 and before April 11, 2023; and on or after October 31, 2023 are ineligible.*
- Oklahoma: Poultry and poultry products derived from birds originating from the State of Oklahoma slaughtered on or after April 9, 2022 and before June 8, 2022 are ineligible.*
- Oregon: Poultry and poultry products derived from birds originating from the State of Oregon slaughtered on or after April 26, 2022 and before March 31, 2023; and on or after September 29, 2023 are ineligible.*
- Pennsylvania: Poultry and poultry products derived from birds originating from the State of Pennsylvania slaughtered on or after March 25, 2022 and before August 29, 2023 are ineligible.
- South Carolina: Poultry and poultry products derived from birds originating from the State of South Carolina slaughtered on or after April 8, 2020 and before August 5, 2020 are ineligible.
- South Dakota: Poultry and poultry products derived from birds originating from the State of South Dakota slaughtered on or after February 12, 2022 and before August 17, 2023, and on or after September 13, 2023 are ineligible.
- Tennessee: Poultry and poultry products derived from birds originating from the State of Tennessee slaughtered on or after August 25, 2022 and before June 10, 2023 are ineligible.
- Texas: Poultry and poultry products derived from birds originating from the State of Texas slaughtered between March 12, 2022 and May 19, 2022, and between December 28, 2022 and February 18, 2023 are ineligible.
- Utah: Poultry and poultry products derived from birds originating from the State of Utah slaughtered on or after April 4, 2022 and before April 22, 2023; and on or after September 15, 2023 are ineligible.*
- Virginia: Poultry and poultry products derived from birds originating from the State of Virginia slaughtered on or after December 29, 2022 and before May 8, 2023 are ineligible.*
- Washington: Poultry and poultry products derived from birds originating from the State of Washington slaughtered on or after April 14, 2022 and before May 27, 2023; and on or after October 6, 2023 are ineligible.*
- Wisconsin: Poultry and poultry products derived from birds originating from the State of Wisconsin slaughtered on or after February 21, 2022 and before April 19, 2023; and on or after November 1, 2023 are ineligible.*
A. Beef Products: PHIS will generate FSIS Form 9060-5, Meat and Poultry Export Certificate of Wholesomeness.
B. Poultry Products: The export applicant will select the statements below via the PHIS statements module when completing the export application in PHIS. The selected statements will be added by PHIS to FSIS Form 9060-5B.
1. The entire consignment of meat comes from poultry;
- which have been kept in State (name of state) free from infection with high pathogenicity avian influenza viruses in poultry since they were hatched or for at least the past 21 days;
- which have been slaughtered in an approved abattoir in State (name of state) free from infection with high pathogenicity avian influenza viruses in poultry and have been subjected to ante-and post-mortem inspections and have been found free of any signs suggestive of avian influenza.
All federally inspected establishments are eligible to export to Benin.
