Export certificates are now processed through PHIS.  Required certificate statements must be added to the export certificate via the PHIS statements module.

1. Alabama: Poultry and poultry products derived from birds originating from the State of Alabama slaughtered on or after October 6, 2023 and before January 16, 2024 are ineligible.
2. Alaska: Poultry and poultry products derived from birds originating from the State of Alaska slaughtered on or after September 14, 2022 and before April 10, 2023 are ineligible.
3. Arkansas: Poultry and poultry products derived from birds originating from the State of Arkansas slaughtered on or after August 1, 2022 and before December 31, 2022; and on or after October 10, 2023 and before March 21, 2024 are ineligible.
4. Arizona: Poultry and poultry products derived from birds originating from the State of Arizona slaughtered on or after October 23, 2024.*
5. California: Poultry and poultry products derived from birds originating from the State of California slaughtered on or after August 1, 2022 and before June 1, 2023; and on or after October 5, 2023 and before June 28, 2024; and on or after August 28, 2024 are ineligible. 
6. Colorado: Poultry and poultry products derived from birds originating from the State of Colorado slaughtered on or after March 18, 2022 and before August 23, 2023; and on or after November 9, 2023 and before March 25, 2024; and on or after June 17, 2024 and before September 10, 2024 are ineligible.

9. Georgia: Poultry and poultry products derived from birds originating from the State of Georgia slaughtered on or after October 31, 2023 and before December 27, 2023 are ineligible.
10. Idaho: Poultry and poultry products derived from birds originating from the State of Idaho slaughtered on or after March 24, 2022 and before March 23, 2023; and on or after November 24, 2023 are ineligible.
11. Illinois: Poultry and poultry products derived from birds originating from the State of Illinois slaughtered on or after November 9, 2022 and before May 4, 2023 are ineligible; and on or after October 29, 2024.*
12. Indiana: Poultry and poultry products derived from birds originating from the State of Indiana slaughtered between January 18, 2022 and July 20, 2022 and between August 11, 2022 and April 18, 2023; and on or after January 4, 2024 and before April 4, 2024 are ineligible.
13. 36. South Carolina: Poultry and poultry products derived from birds originating from the State of South Carolina slaughtered on or after April 8, 2020 and before August 5, 2020 are ineligible.
37. South Dakota: Poultry and poultry products derived from birds originating from the State of South Dakota slaughtered on or after February 12, 2022 and before August 17, 2023, and on or after September 13, 2023 and before June 10, 2024 are ineligible.
38. Tennessee: Poultry and poultry products derived from birds originating from the State of Tennessee slaughtered on or after August 25, 2022 and before June 10, 2023 are ineligible.
39. Texas: Poultry and poultry products derived from birds originating from the State of Texas slaughtered on or after March 12, 2022 and before May 19, 2022, and on or after December 28, 2022 and before February 18, 2023, and on or after March 12, 2024 and before July 15, 2024 are ineligible.
40. Utah: Poultry and poultry products derived from birds originating from the State of Utah slaughtered on or after April 4, 2022 and before April 22, 2023, and on or after September 15, 2023 and before December 14, 2023, and on or after September 24, 2024 are ineligible.
41. Virginia: Poultry and poultry products derived from birds originating from the State of Virginia slaughtered on or after December 29, 2022 and before May 8, 2023 are ineligible.
42. Washington: Poultry and poultry products derived from birds originating from the State of Washington slaughtered on or after April 14, 2022 and before May 27, 2023; and on or after October 6, 2023 and before May 14, 2024, and on or after September 24, 2024 are ineligible.
43. Wisconsin: Poultry and poultry products derived from birds originating from the State of Wisconsin slaughtered on or after February 21, 2022 and before April 19, 2023; and on or after November 1, 2023 and before March 22, 2024 are ineligible.

A. Beef Products: PHIS will generate FSIS Form 9060-5, Meat and Poultry Export Certificate of Wholesomeness.

B. Poultry Products: The export applicant will select the statements below via the PHIS statements module when completing the export application in PHIS.  The selected statements will be added by PHIS to FSIS Form 9060-5B. 

1. The entire consignment of meat comes from poultry; 

1. which have been kept in State (name of state) free from infection with high pathogenicity avian influenza viruses in poultry since they were hatched or for at least the past 21 days;
2. which have been slaughtered in an approved abattoir in State (name of state) free from infection with high pathogenicity avian influenza viruses in poultry and have been subjected to ante-and post-mortem inspections and have been found free of any signs suggestive of avian influenza.

All federally inspected establishments are eligible to export to Benin.
