       1. on or after July 14, 2022 and before March 23, 2023; and

    1. Alexandria County on or after February 23, 2023 and before May 26, 2023; and
    2. Rockingham County on or after January 19, 2023 and before July 9, 2023 are ineligible for export to Dominica.
23. Washington
    1. Franklin County

       2. on or after October 15, 2024;

       1. on or after June 7, 2022 and before February 27, 2023; and
       2. on or after December 19, 2023 and before July 15, 2024;

    5. Snohomish County on or after July 18, 2022 and before July 28, 2023 are ineligible for export to Dominica.
24. Wisconsin
    1. Barron County
       1. on or after April 12, 2022 and before September 26, 2022; and
       2. on or after December 4, 2023 and before April 23, 2024;
    2. Dunn County on or after September 29, 2022 and before February 2, 2023;
    3. Jefferson County
       1. on or after March 14, 2022 and before August 23, 2022; and
       2. on or after November 7, 2022 and before June 20, 2023;
    4. Polk County on or after April 15, 2022 and before September 1, 2022;
    5. Racine County on or after April 7, 2022 and before March 2, 2023;
    6. Trempealeau County on or after November 22, 2023 and before April 21, 2024;
    7. Washburn County on or after January 5, 2024 and before May 23, 2024; and
    8. Washington County on or after September 2, 2022 and before April 30, 2023 are ineligible for export to Dominica.

All export certificates accompanying product to Dominica may be signed by either an FSIS veterinarian or inspector.

All export applications for meat and poultry exports must be entered and processed through PHIS beginning Monday, March 6, 2023. 

Effective for all meat and poultry export certificates approved, signed, and issued on or after March 6, 2023, all new and existing FSIS Letterhead Certificates or statements previously required in the 9060-5 Remarks block 17 will no longer be signed and issued by FSIS IPP. Instead, the export applicant will select the equivalent statements via the PHIS statements module when completing the export application in PHIS. The selected statements will be added by PHIS in the remarks continuation page, FSIS Form 9060-5B: Meat, Poultry and Egg Products Certification of Wholesomeness Continuation Sheet. 

1. Poultry and poultry products - FSIS Form 9060-5: Meat and Poultry Export Certificate of Wholesomeness is required and will be generated through PHIS. 
   
   NOTE: For exports certified on or after March 6, 2023, the statements below must be added to the export certificate via the statements module when completing the export application in PHIS. The selected statements will be added by PHIS in the remarks continuation page, FSIS Form 9060-5B. 
   1. Fresh poultry meat derived from birds originating in the United States or Canada or was imported in accordance with conditions meeting recommendations of the OIE for listed diseases affecting poultry transmissible through the product.
   2. In compliance with the United States' National Residue Program, the meat does not contain residues of antibiotics or their metabolites in quantities exceeding U.S. established tolerance levels. Growth-promoting hormones are not approved for use in poultry in the United States.
   3. The birds giving origin to the product to be shipped to the Commonwealth of Dominica were under State supervision for the diseases regulated by the State and/or the United States Department of Agriculture and presented no evidence of avian influenza. Alternatively, the meat product was legally imported into the U.S.
   4. The United States or county of origin has been free of both highly pathogenic avian influenza and velogenic Newcastle disease for at least 90 days. Farms of origin are free of notifiable avian influenza. The slaughter plants and flocks of origin of chicken and turkey meat are approved for export based on compliance with the standards of U.S. H5/H7 Avian Influenza Monitored. Alternatively, the meat product was legally imported into the U.S.
2. Beef and beef products - FSIS Form 9060-5: Meat and Poultry Export Certificate of Wholesomeness is required and will be generated through PHIS. 
   
   NOTE: For exports certified on or after March 6, 2023, the statements below must be added to the export certificate via the statements module when completing the export application in PHIS. The selected statements will be added by PHIS in the remarks continuation page, FSIS Form 9060-5B. 
   1. The animals from which the meats originate come from healthy herds under State supervision for the diseases regulated by the State and/or the United States Department of Agriculture or the animals/meat products were legally imported.
   2. The United States complies with the conditions referred to in the Article on bovine spongiform encephalopathy in the Terrestrial Animal Health Code of the OIE as a country posing a "negligible risk" for bovine spongiform encephalopathy.
   3. The bovine meat and meat products were subject to testing in accordance with the U.S. National Residue Program.
3. Pork and pork products - FSIS Form 9060-5: Meat and Poultry Export Certificate of Wholesomeness is required and will be generated through PHIS. 
   
   NOTE: For exports certified on or after March 6, 2023, the statements below must be added to the export certificate via the statements module when completing the export application in PHIS. The selected statements will be added by PHIS in the remarks continuation page, FSIS Form 9060-5B. 
   1. The meat was derived from animals born in the United States or imported from countries or regions which have been evaluated and found to be free of the following diseases: African swine fever, foot-and-mouth disease, classical swine fever, rinderpest, and swine vesicular disease. These diseases have not occurred in the United States of America during the last twelve (12) months.
   2. The animals from which the meats originate come from healthy herds under State supervision for the diseases regulated by the State and/or the United States Department of Agriculture, including OIE listed diseases which affect the species and may be carried through meat, and were not slaughtered as a result of any infectious, contagious, or parasitic disease eradication program. The meat is deemed by USDA to not pose a risk of transmission of brucellosis or tuberculosis.
   3. The meat and meat products were produced under mandatory HACCP regulations and the U.S. National Residue Program and were found to be in full compliance.
   4. The establishments where animals are slaughtered and processed for export are inspected by USDA FSIS.
   5. The pork and pork products were derived from carcasses that:
      1. tested negative for trichinosis; or
      2. were subject to a freezing process or other mitigation process that guarantees the destruction of the parasite Trichinella spiralis.
4. Sheep/goat meat and sheep/goat meat products - FSIS Form 9060-5: Meat and Poultry Export Certificate of Wholesomeness is required and will be generated through PHIS. 
   
   NOTE: For exports certified on or after March 6, 2023, the statements below must be added to the export certificate via the statements module when completing the export application in PHIS. The selected statements will be added by PHIS in the remarks continuation page, FSIS Form 9060-5B. 
   1. The animals from which the meats originate come from healthy herds under State supervision for the diseases regulated by the State and/or the United States Department of Agriculture or the animal/ meat and products were legally imported.
   2. The United States has established a system for the surveillance and monitoring of scrapie and any animals which are found to be affected are excluded from slaughter.
   3. The establishment from which the meats originate is under inspection by USDA, FSIS, or were legally imported into the U.S.
   4. Based on the National Residue Program, the meats do not contain residues or metabolites of antibiotics, growth promoters, pesticides or prophylactic or therapeutic drugs at levels which exceed the tolerance limits established.

All Federally inspected plants are eligible for export to the Commonwealth of Dominica.
