Export Library - Requirements by Country
NOTE: Effective June 29, 2018, all meat (including Siluriformes) and poultry exports to the following 16 countries must be processed through PHIS (Public Health Information System): Afghanistan, Andorra, Bahamas, Bolivia, Burundi, Cape Verde, Cook Islands, Ethiopia, French Guiana, Gambia, Guinea, Liberia, Mozambique, San Marino, Tanzania, and Uganda. These countries do not maintain additional export requirements and therefore do not appear in the country list below.
NOTE: Effective May 20, 2019, all meat (including Siluriformes) and poultry exports to the following 21 countries must be processed through PHIS (Public Health Information System): Angola, Antarctica, Aruba, Bhutan, Botswana, Bouvett Island, Brunei Darussalam, Christmas Island, Comoros, Eritrea, Falkland Islands, Faroe Islands, French Southern and Antarctic Islands, Greenland, Guernsey, Heard Island and McDonald Islands, Isle of Man, Jersey, Lesotho, Liechtenstein, and Vietnam. Most of these countries do not maintain additional export requirements and therefore do not appear in the country list below.
On May 20, 2019, all export applications for all meat (including Siluriformes) and poultry exports to the 37 countries listed above must be processed through PHIS. Applicants are to complete the electronic export application through PHIS. For those applicants not using PHIS, they are to submit a paper export application, FSIS Form 9060-6 (available here or from the forms tab of the FSIS website) to the FSIS Proxy by one of the following methods:

NOTE: Using fax to send the completed application (FSIS Form 9060-6) to FSIS is only permitted when no additional supplemental documents (e.g. letterhead certificates) are required by the importing country as specified in the FSIS Export Library. If the application includes required supplemental documents (e.g. letterhead certificates) the application and associated documents must be sent by email or physical mail to the designated address above.
If an export applicant has questions about sending the export application and associated documents to the FSIS Proxy, they can call 515-334-2000 and select option 8.

Note: When signing export certificates for countries, if there are questions regarding the avian influenza statement on the certificate please call FSIS at (202) 720-0082 or (855) 444-9904 for clarification. FSIS continues to strongly encourage exporters to work closely with importers to ensure the existence of a valid import permit for product in the country of destination.
If a country is not listed, it should be assumed that nothing is known about that country's import requirements, and FSIS will issue the FSIS Form 9060-5, Meat and Poultry Export Certificate of Wholesomeness, without any additional information provided in the "Remarks" section. For a country that is not listed, exporters are advised to work closely with the importer for information regarding eligibility of the product, certification requirements, and the existence of a valid import permit for the product.

Export Requirements for Meat and Poultry Products

Siluriformes (catfish) Export Eligibility
Siluriformes (catfish) derived fresh, frozen, and processed products are eligible for export by following the same policy for exporting meat and poultry products when a country or FSIS amenable product is not listed in the export library. After performing re-inspection of the product, FSIS inspection program personnel will issue the FSIS Form 9060-5S, Siluriformes Fish and Fish Products Export Certificate of Wholesomeness, to the exporter without any additional information provided in the “Remarks” section. Visit the specific country’s export library page to obtain the most up-to-date information regarding the individual country requirements for exporting Siluriformes products.
Instructions for Completing the Export Certificate of Wholesomeness
Beginning on June 29, 2018, FSIS IPP will certify all meat and poultry exports on the revised Meat and Poultry Export Certificate of Wholesomeness (FSIS Form 9060-5) and IPP will certify all Siluriformes Fish exports on the revised Siluriformes Fish and Fish Products Export Certificate of Wholesomeness. Using the instructions for completing FSIS Form 9060-5 or FSIS Form 9060-5S, the export applicant is to complete the revised paper certificate prior to presenting to FSIS IPP for review and certification (signature and date). The revised FSIS Form 9060-5 will begin with the MPM series and the revised FSIS Form 9060-5S will begin with the CFB series.
