1. Poultry and poultry products

1. FSIS Form 9060-5 must be signed by a FSIS veterinarian.
   NOTE: The export applicant will select the equivalent statement via the PHIS statements module when completing the export application in PHIS. The selected statement will be added by PHIS to FSIS Form 9060-5B.
2. For poultry and poultry products, the following statement must be added to the export certificate via the statements module.

"The poultry meat and poultry meat products have been sourced from a zone free of Newcastle disease and highly pathogenic avian influenza (as both diseases are defined by the World Organization for Animal Health) in commercial poultry." *
